How to Submit a Public Records Request

Effective April 19, 2023, to request a public record from the Alabama Medicaid Agency you must download and complete a public records request form and submit it as an attachment through email to:  public.records@medicaid.alabama.gov.

Please note that this form is only for requesting public records from the Alabama Medicaid Agency. If you are looking for public records from another governmental entity, you must send your request directly to that entity. The Alabama Medicaid Agency will not forward or otherwise process such requests.

In addition, the Alabama Medicaid Agency is permitted by state law to charge fees and costs in connection with processing public records requests. Of course, the Agency will notify you in advance of the fees you may incur as a result of your request.
