All subpoena requests must be made out to the Alabama Medicaid Agency and submitted to the attention of Office of General Counsel located at 501 Dexter Avenue, Montgomery, AL 36104. The subpoena must include HIPAA compliant language or a HIPAA Order filed in court. Please include instructions filed at the time of issuance on whom and what records you are requesting.
