Alabama Coordinated Health Network (ACHN)

Alabama Coordinated Health Network (ACHN) is an innovative plan to transform health care provided to Medicaid recipients in Alabama. The ACHN Program transforms the Alabama Medicaid delivery system into a more flexible and cost-efficient effort. This program is designed to create a single coordinated care management delivery system that effectively links patients, providers and community resources in each of the seven regions. Delivery of medical services is not part of this program.
