RE:      2024 SNF Cost Report – Establish Submission Portal Account

Effective for the June 30, 2024 cost report (due September 30, 2024) and going forward, all cost reports will be submitted through a new and secure web portal. This web portal will be hosted by Myers and Stauffer, who will process your facility’s submitted registration form.

A link to the registration form has been added to the “Forms” posted on the Skilled Nursing Facility Reimbursement webpage on Medicaid’s website.

The web portal registration form is necessary to ensure all required contacts for your facility have access to the portal and relevant cost reporting and rate setting files. The registration form asks for the following items:

• Facility Legacy Number (or Federal Employee ID Number for Home Offices)* 
• Medicaid Number
• Medicare Number 
• NPI
• Provider Name
• Entity Type (Home Office or Facility)
• Contact Name
• Contact Job Title
• Contact Email Address
• Contact Phone Number
• Contact User Role (see registration form for more details on user roles)
• Third Party Verification

* The Legacy Number is the number that your facility used to submit previous years’ cost reports and is formatted as 47-XXXX0S.
 
You may submit a single registration form for multiple providers/home offices, if needed.
 
All registration forms must be signed by the appropriate CEO/CFO or owner, and an additional authorization is required for all registrations for third-party contacts. Should you have any changes in contacts, it is your responsibility to notify Myers and Stauffer as soon as possible so your facility’s contact listing can be updated, and ensure all necessary parties have access to the portal.

The registration form should be submitted to Myers and Stauffer at ALCR@mslc.com no later than August 30, 2024 to have access at the beginning of September. All registration forms take 1-2 business days to process, so please ensure you register and validate your access to the web portal in advance of your submission deadline.
