An Integrated Care Network (ICN) is designed to be a competitively bid, integrated network to provide enhanced case management, education and outreach services to most Medicaid long term care recipients in both HCBS and institutional settings. The Integrated Care Network (ICN) program was approved by the Centers for Medicare and Medicaid Services (CMS) on September 12, 2018, to offer more community options for Medicaid long-term care recipients. The ICN supports the Primary Care Case Management (PCCM) Entity model.
