Alabama Coordinated Health Network (ACHN)

On June 14, 2019, CMS approved the 1915(b) waiver which allows Alabama Medicaid to implement the Alabama Coordinated Health Network (ACHN)!

ATTENTION Primary Care Physicians:  The ACHN program will be implemented on October 1, 2019, and providers need to complete an ACHN Agreement ASAP in order to receive bonus and participation payments. The end date for the Patient 1st Program is approaching, and providers will be required to have completed agreements with both Medicaid and the ACHN. PCPs will NOT be receiving a capitated payment in October of 2019.

ACHN is an innovative plan to transform health care provided to Medicaid recipients in Alabama. ACHN transforms the Alabama Medicaid delivery system in to a more flexible and cost-efficient effort. This effort will build off of the Agency’s current case management program structure.

Important note: This program is designed to create a single care coordination delivery system that effectively links patients, providers and community resources in each of the seven newly-defined regions. Delivery of medical services is not part of this program.
