Alabama Medicaid announces 1095-B Form Process

Again, this year, Alabama Medicaid will not be mailing a 1095-B form to recipients.

Due to a change in law about three years ago, most Medicaid recipients do not need proof of Medicaid coverage to file their taxes.

However, recipients may still request their 1095-B form by telephone, e-mail, or postal mail. That contact information is as follows:

Please include your first and last name, Medicaid number, current mailing address on file, and/or new mailing address.

Alabama Medicaid can only send a 1095-B form to recipients who had full Medicaid or pregnancy Medicaid in the past five years.
