Targeted Case Management (TCM) services are available to assist Medicaid recipients in eight specific groups to gain access to needed medical, social, educational, or other services. TCM services are only provided to recipients in home or residential settings. Patients in hospitals, nursing facilities, prisons, or other total care facilities do not qualify for TCM services.
Target Groups
- Mentally ill adults - Medicaid-eligible individuals age 18 and over who have been diagnosed with mental illness - TCM services provided by the Alabama Department of Mental Health
- Intellectually disabled adults - Medicaid-eligible individuals age 18 and over who have been diagnosed with an intellectual disability - TCM services provided by the Alabama Department of Mental Health
- Handicapped children - Medicaid-eligible children age 0-21 who are considered handicapped - TCM services provided by the Alabama Department of Mental Health
- Foster children - Medicaid-eligible children aged 0-21 who are in the care, custody or control of the state of Alabama - TCM services provided by the Alabama Department of Human Resources
- Pregnant women - Medicaid-eligibile women of any age in need of maternity services - TCM services administered by Alabama Medicaid Maternity Program
- AIDS/HIV-positive individuals - Medicaid-eligible individuals of any age who have been diagnosed as having AIDS or being HIV-positive - TCM services administered by the Alabama Department of Public Health
- Adult protective service individuals - Medicaid-eligible individuals age 18 or over who are at risk of abuse, neglect or exploitation - TCM services provided by the Alabama Department of Human Resources
- Technology Assisted (TA) Waiver for Adults - Medicaid-eligible individuals age 21 or over who meet the eligibility criteria for the TA Waiver - TCM services administered by Alabama Medicaid Program
