Provider Survey to Assess Substance Use Disorder Treatment Needs in Alabama

TO: Prescribers / Primary Care Providers

The Alabama Medicaid Agency (Medicaid) and the Alabama Department of Mental Health (ADMH) are assessing the current capacity of Alabama’s healthcare community for identifying and treating individuals with substance use disorders and finding ways to enhance substance use treatment and recovery options in our state. We need your assistance in providing information about your primary care practice and how you typically handle substance use concerns. The information you provide will be combined with information from other providers to identify capacity and needs across the state, within regions, and within counties. The University of Alabama is conducting this project for Medicaid and ADMH.

 

The survey will take 12-15 minutes, and all the information you provide will be kept completely confidential. Please complete the survey within two weeks.
